Baptist Health Care Foundation Selected for Three-Year, $300,000 Florida Blue Foundation Grant for Faith Health Network

PENSACOLA, Fla. (Feb. 18, 2020) – The Baptist Health Care Foundation was recently awarded a three-year, $300,000 grant from Florida Blue Foundation to support Baptist Health Care’s Faith Health Network (FHN). The grant will allow FHN to create an innovative community care management program called Aligning Care Across Escambia.

“We are grateful to Florida Blue Foundation for its support of the work of Faith Health Network,” said KC Gartman, chief development officer for Baptist Health Care. “This funding allows for us to improve patient care and reduce the need for medical services by enhancing coordination of care, eliminating duplication and helping patients and caregivers more effectively manage health conditions.”

The program is designed to support the most frequent users of Baptist Hospital’s emergency department to discover new ways to improve health care utilization. Basic screening tests will be performed on a quarterly event basis, beginning with Baptist Health Care’s FHN congregations, and will utilize results to connect members to needed health and social resources.

“The program goal is to create and test the community health and wellness concept,” said Joy Sharp, community health program director. “This concept will include wellness, mentoring, advocacy and education within and for the community. The ultimate goal is to define a collaborative hospital and community strategy that can be replicated in other neighborhoods and communities.”
